The International Collaboration on Oropharyngeal cancer Network for Staging (ICON-S) system was superior to the MD Anderson Cancer Center (MDACC) and American Joint Committee on Cancer seventh edition (AJCC-7ed) systems for survival stratification of patients with human papillomavirus (HPV)-related oropharyngeal squamous cell carcinoma (OPSCC).
Why this matters
The AJCC-7ed does not stratify based on HPV status, despite superior survival outcomes of HPV+ patients.
Survival was not significantly associated with AJCC-7ed stage (OS HR, 1.1; P=.95 for stage III, HR, 3.5; P=.22 for stage IV; DFS HR, 0.88; P=.91 for stage III, HR, 1.8; P=.56 for stage IV).
Survival was associated with ICON-S stage (OS HR, 3.3; P=.002 for stage III, HR, 18; P<.001 for stage IV; DFS HR, 2.9; P<.001 for stage III, HR, 6.5; P=.011 for stage IV).
